Früher Einsatz der Kombination Betablocker und Ivabradin bei der Herzinsuffizienz-Behandlung führt zur Reduktion der Sterblichkeit, Verbesserung der Symptomatik und Steigerung der Leistungsfähigkeit

Abstract

Hintergrund: Zahlreiche Mortalitäts- und Morbiditätsstudien haben gezeigt, dass die Höhe der Herzfrequenz mit der Herzinsuffizienz-Prognose direkt korreliert.Deshalb werden bisher bei der Therapie der Herzinsuffizienz vor allem Betablocker eingesetzt. Dennoch ist die Herzfrequenzkontrolle durch Betablocker oft nicht ausreichend, weil diese Substanzen wegen zusätzlicher kardialer Effekte oder anderer Nebenwirkungen häufig nicht bis zur Zieldosis auftitriert werden können. Mit Ivabradin steht eine effektive Substanz für die Therapie der Herzinsuffizienz zur Verfügung, welche ausschließlich die Herzfrequenz senkt, ohne weitere kardiale Parameter zu beeinflussen. Die logische Konsequenz ist der Einsatz der Kombination Betablocker und Ivabradin als optimale und effektive Therapieoption bei chronischer Herzinsuffizienz.

Prognostische Wirksamkeit: In der SHIfT-Studie (n = 6.505) reduzierte die Kombinationstherapie aus Betablocker und Ivabradin bei Herzinsuffizienz-Patienten den primären Endpunkt (kardiovaskuläre Mortalität plus stationäre Einweisung wegen Herzinsuffizienz) signifikant um 18 %. Besonders ausgeprägt war der Behandlungseffekt bei Patienten mit einer Ausgangs-Herzfrequenz ≥ 75 Schlägen/Minute: Die Kombination senkte in dieser Population die kardiovaskuläre Sterblichkeit und die Gesamt-Mortalität signifikant um jeweils 17 %, die Herzinsuffizienz-Mortalität um 39 %. Der prognostische Vorteil korrelierte dabei direkt mit der nach 28 Tagen erreichten Reduktion der Herzfrequenz.

Symptomatische Wirksamkeit: Die früh im Therapieverlauf bei Herzinsuffizienz eingesetzte Kombination aus Betablocker und Ivabradin senkte bei Patienten mit einer Ausgangs-Herzfrequenz von ≥ 75 Schlägen/Minute die stationären Einweisungen wegen Herzinsuffizienz um 30 %, reduzierte die Symptomatik und erhöhte die Lebensqualität. Im Vergleich zu einer Betablocker-Monotherapie verlängerte die Kombination Betablocker und Ivabradin die 6-Minuten-Gehstrecke signifikant und erhöht somit die Leistungsfähigkeit herzinsuffizienter Patienten.

Ventrikuläres Remodelling: Die Kombination Betablocker und Ivabradin kehrt das kardiale Remodelling um: Das endsystolische linksventrikuläre Volumen nimmt ab und die Ejektionsfraktion steigt.

Schlussfolgerungen: Der frühe Einsatz der Kombination Betablocker und Ivabradin in der Therapie der Herzinsuffizienz verbessert aufgrund der effektiven Reduktion der Herzfrequenz die Prognose von Herzinsuffizienz-Patienten und reduziert deutlich die Symptomatik: Stationäre Einweisungen wegen Herzinsuffizienz sowie kardiovaskuläre Sterblichkeit und Gesamt-Mortalität nehmen signifikant ab, Leistungsfähigkeit und Lebensqualität steigen.

Early treatment with the combination beta-blocker plus ivabradine in heart failure treatment results in reduced mortality, improves signs and symptoms und increases exercise capacity

Abstract

Background: Several mortality and morbidity studies have demonstrated direct correlations between heart rate and prognosis of heart failure patients. Therefore, beta-blockers belong to the mainstay of heart-failure treatment. However, control of heart failure by beta-blockers is often not sufficient as these compounds cannot be titrated up to the target dose due to additional cardiac effects or other side-effects. With ivabradine, an effective drug for heart failure treatment is available which exclusively lowers the heart rate without interfering with other cardiac parameters. The logical consequence is the use of ivabradine in combination with beta-blocker as an optimal and effective treatment option in chronic heart failure.

Prognostic efficacy: In the SHIfT-study (n = 6505), combination treatment with ivabradine plus beta-blocker reduced in heart failure patients the combined primary endpoint (cardiovascular mortality plus hospital admission due to heart failure) significantly by -18%. The treatment effect was especially pronounced in patients with a baseline heart rate ≥75 beats/minute: In this population, the combination reduced the overall mortality and the cardiovascular mortality significantly by -17 % each and the heart failure mortality by -39 %. The prognostic advantage correlated directly with the heart rate reduction achieved after 28 days.

Symptomatic efficacy: The combination beta-blocker plus ivabradine administered early in the treatment course lowered in patients with a baseline heart rate of ≥75 beats/minute hospital admissions due to heart failure by -30%, reduced signs and symptoms and increased the quality of life. Compared with beta-blocker monotherapy, the combination beta-blocker plus ivabradine extended the 6-minutes walking distance und consequently increased the exercise capacity of patients with heart failure.

Ventricular remodelling: The combination beta-blocker plus ivabradine reverses cardiac remodelling: The end-systolic left-ventricular volume decreases und the ejection fraction increases.

Conclusions: The early administration of the combination beta-blocker plus ivabradine in the treatment of heart failure improves the prognosis of heart failure patients und reduced the symptoms considerably due an effective reduction of the heart rate: Hospital admissions because of heart failure as well as cardiovascular mortality and overall mortality decrease significantly, and exercise capacity as well as quality of life increase.
